3. Emergency Services
The Health Proxy does not provide emergency medical services.
If you are experiencing a medical emergency, call 911 immediately or go to your nearest emergency department.

4. Client Responsibilities
Clients agree to:
Provide accurate and current personal and insurance information.
Notify us of any changes to appointments or healthcare coverage.
Participate in decisions regarding their healthcare.
Provide any required authorizations needed for us to communicate with healthcare providers, insurance companies, or community organizations on their behalf.
5. Authorization to Communicate
With the client's written permission, The Health Proxy may communicate with healthcare providers, specialists, hospitals, pharmacies, insurance companies, family members, caregivers, and community organizations as necessary to provide advocacy and care coordination services.
Clients may revoke authorization at any time in writing.
6. Privacy
Protecting your privacy is important to us. We take reasonable administrative, technical, and physical safeguards to protect the personal information you share with us.
Information is collected and used only as necessary to provide our services, comply with applicable laws, and support healthcare coordination.

7. Medicare Billing
The Health Proxy provides services that are billed through Medicare when eligible and authorized under applicable Medicare guidelines.

Coverage and reimbursement decisions are made solely by Medicare and any applicable payer. While we make every reasonable effort to submit accurate information, we cannot guarantee that every service will be covered or reimbursed.
If a service is not covered by Medicare or requires additional authorization, we will discuss available options with the client whenever possible before services are provided.
8. No Guarantees
While The Health Proxy is committed to providing high-quality advocacy and care coordination services, we cannot guarantee:
Appointment availability
Insurance approval or coverage decisions
Acceptance by healthcare providers or specialists
Acceptance into assistance or community programs
Specific healthcare outcomes
Medical results or treatment success
Healthcare decisions remain the responsibility of licensed healthcare providers and insurance organizations.
9. Intellectual Property
All content on this website, including text, graphics, logos, educational materials, downloads, and other content, is the property of The Health Proxy unless otherwise stated.
